• Fill in all the
information in the top portion of
the form
• Under the Player Information column only enter “Name” if you are adding last names to the back of the jerseys
• Please sort players by jersey numbers in ascending order from 1 to 99
• Under the Size Information columns, please enter jersey size, short size, and sock size. Socks may be ordered in Youth, or Adult Large sizes.
• At the bottom of the form, please total the number of jerseys, shorts and socks ordered, both in total ordered and then by each size.
• The form may be emailed to Kathy Ortiz or mailed to:

• Checks are made payable to “SASC” and sent to Kathy Ortiz at the address noted above.

The cost of a complete SASC uniform package is $65.00. This includes:
• Two jerseys, home and away
• One pair of shorts
• Two pairs of socks
• One practice t-shirt
Last names may be added to the back of the jerseys at a cost of $6.00 per jersey. Teams may personalize either jersey, or both home and away jerseys.
Uniform pieces may be purchased separately as listed below:
• Jersey $20.00
• Shorts $13.00
• Socks $6.00
• Practice t-shirt $6.00
