Medina Fall Classic Tournament
The 2010 Medina Fall Classic Tournament is a US Club Soccer sanctioned tournament for U11 through U16 girls and boys teams. This is a Silver level tournament for teams registered with US Club Soccer credentials. CYSA and other USYS registered teams welcome. All matches will be played on the beautiful soccer fields of Sunnyvale and Cupertino, California.

The Tournament Committee would like to extend a welcome to this year's participants.

A game used ball used from the September 29th game between the San Jose Earthquakes and Chicago Fire will be raffled off at this weekend's Medina Tournament.

Insurance for non-US Club Soccer teams: Accepted teams that are not registered as US Club Soccer teams must purchase insurance at a cost of $50 per team to cover the team for the entire weekend for tournament play. To learn more about and purchase tournament team insurance click here. Accepted teams that are not registered as US Club Soccer teams must email proof of insurance to tournament@sunnyvalesoccerclub.org no later than October 15th, 2010. Accepted teams that are not registered as US Club Soccer teams, that fail to email proof of insurance on or before Oct 15th, 2010 OR fail to display their proof on insurance during credential check-in will not be able to play in the tournament and will forfeit their tournament Application Fee.

Credential Check-in Requirements:
  • Complete your Team Roster in GotSoccer by 5pm, Wednesday, Oct, 27th.
  • Include All Guest Players in GotSoccer Roster
  • Each Player MUST have
    • Player Name
    • Player ID# of the Pass being used in the Tournament (US Club or CYSA)
    • Player Jersey #
  • GotSoccer Team Roster is printed directly on Match Roster Cards
  • Have roster stickers available for play-off matches.

US Club Soccer Registered Teams:
  • Current US Club Soccer Official Team Roster
    • MUST list all GUEST players as well
  • Current laminated US Club Soccer Passcards for all Players and Staff
  • Completed and Signed Registration/Medical Release form for every Player
  • For Teams guesting players loaned from another CLUB
    • Make sure you have submitted the Player Loan Form to US Club Soccer
    • This MUST be mailed to US Club Soccer PRIOR to the event.

CYSA/USYSA Registered Teams:
  • Current CYSA Official Team Roster (goldenrod) or USYSA equivalent
  • Current laminated USYSA member passes for all Players and Staff
  • Completed and Signed USYSA Registration Form for every Player
    • For CYSA, this is form 1601
  • Completed and Signed Guest Player Form for each Guest Player
    • Available from CYSA in PDF form here.
  • US Club Soccer one-event approved Tournament Team Roster listing all players insured for the Medina Fall Classic Tournament.
    • NOTE: This MUST list all guest players as well.
